Develop Necessary Partnerships
Many people with complex needs require services that are well outside of the bounds of traditional health care structures. Organizations and funding models tend to put critical needs in different silos — whether for behavioral health, dental health, housing, transportation, food security, and social needs, among others. Enhanced care models must have a plan in place to address unmet health-related needs in order to see and sustain improvement. Cultivating partnerships with clinical, social service, and community-based organizations is key to providing better health care for people with complex needs.
