What Care Models Are Promising?

The most promising care models for people with complex needs create interdisciplinary teams that collaborate to meet the person’s needs for primary care, transitions after a hospitalization, palliative care, and/or long-term services and supports. The evidence base on care models for people with complex needs is still evolving, therefore it is important to continue testing models in different settings and contexts to develop better person-centered outcome measures and understand their effects on total cost of care.
